Once your appointment is scheduled, you will receive access to the secure patient portal to complete your intake forms, consent documents, and practice policies.

Attend Your Evaluation
Join your secure telehealth appointment from a private location in Arizona. During your initial evaluation, we will discuss your concerns, history, goals, and treatment options.

Before Your First Appointment
- A valid photo ID
- A current list of medications, supplements, and allergies
- Your preferred pharmacy’s name and location
- Relevant psychiatric or medical records, testing results, or previous medication history, if available
- A private, quiet location in Arizona with reliable internet and a device with a working camera and microphone
